Complete your BPI Card Application in 2 EASY STEPS!

Important: Application will be cancelled if documents are not received within 3 banking days.

STEP 1. Prepare a photocopy of the following documents:

a. Proof of ID (Front and back of at least one (1) valid ID with picture)

Passport
Voter’s ID
Postal ID
SSS/GSIS ID
BIR/TIN ID
Driver’s License
Professional Regulation Commission (PRC) ID
Company ID issued by private entity or institutions registered with or supervised and regulated by the BSP, SEC or IC

b. Proof of Income

For Employed		For Self Employed
Any of the following documents: - Certificate of Employment - Latest two (2) pay slips - Latest Income Tax Return (ITR) with BIR/Bank stamp		Latest audited financial statements (AFS) with BIR/bank stamp AND Latest Income Tax Return (ITR) with BIR/Bank stamp

For Overseas Filipino Workers
SEA-BASED (AGENCY-BASED WORKERS)	LAND-BASED (AGENCY-BASED WORKERS)		DIRECT HIRE
POEA-Validated contract of employment Latest two (2) months proof of remittance	POEA-Validated contract of employment Overseas Employment Certificate or POEA-validated OFW information sheet Latest two (2) months proof of remittance		POEA-Validated contract of employment Overseas Employment Certificate or POEA-validated OFW information sheet Latest two (2) months proof of remittance

c. Additional requirements (if applicable):

If a representative will receive the card on the cardholder’s behalf
- BPI Card Delivery Authorization Form (download here), or
- BPI Family Card Delivery Authorization Form (download here)
Existing SkyMiles Member (any of the following):
- SkyMiles ID
- Any document that indicates the valid and registered name and number in the SkyMiles membership program
Non Filipino Resident
- Alien Certificate of Registration (ACR), work permit, or Embassy Accreditation Papers
Owners/ Managers of Pawnshop, Forex Dealers, Money Changers & Remittance Agents
- Certificate of Registration issued by BSP
Insurance Agents
- Insurance commission license
Existing Credit Cardholder (with other banks)

Principal cardholders for at least one year are no longer required to submit any proof of income if they are able to provide any of the following:

- Photocopy of their credit card or
- Latest credit card statement/s

STEP 2. Submit the documents through any of the following:

Additional Reminders:

1. Applicants must be between 21 to 60 years old.

2. Applicant should have the following minimum fixed monthly income (excluding overtime, commissions, or service fees):

P15,000 for BPI Blue Mastercard, BPI Skymiles Mastercard, Petron-BPI Mastecard, BPI Edge Mastercard, BPI Amore Visa Card

P 40,000 for BPI Gold Mastercard

P 80,000 for BPI SkyMiles Platinum Mastercard, BPI Amore Visa Platinum Card

P 30,000 for Overseas Filipino workers

P 30,000 for the Self-employed

3. Applicants must have a business or residence contact number

4. Applicants must have submitted a completely filled out application form.

5. For applicants who will have an authorized representative receive the card on their behalf, the card delivery authorization form must be presented to the courier upon delivery of card.
